Schwaller Center Coordinator

High Plains Mental Health Center currently has a full-time position opening for a Schwaller Center Coordinator in our Schwaller Center location in Hays. The qualified candidate will develop and coordinate a team of Schwaller Center Recovery Workers who will provide Attendant Care, Intensive Community Support, and intensive case management services to patients to assist in resolving crises or averting crises and deterioration of condition, and assists patients to return to, if possible, their prior living situations and improved level of functioning. This program will serve adult patients of HPMHC who are identified as SPMI and individuals at risk of hospitalization or becoming SPMI. Provide supervision, support and a sense of teamwork to a staff who will be working different shifts in a variety of settings. The qualified candidate will oversee the day-to-day provision of Schwaller Center services provided by the staff and manages to ensure that services result in positive outcomes for patients, through facilitation of team meetings and individual supervision and support of Schwaller Center workers. The qualified candidate will ensure that the team’s work reflects values and attitudes consistent with the agency’s philosophy of consumer choice, a strengths perspective, and recovery oriented practice.

Qualifications include (1) Have at least a BA/BS degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education with one year of experience substituting for one year of education; (2) possess demonstrated interpersonal skills, ability to work with persons with severe and persistent mental illness and/or severe emotional disturbance, and the ability to react effectively in a wide variety of human service situations; (3) meet the specifications outlined in the CMHC licensing standards in regard to any ongoing requirements (as in completion of training requirements according to a curriculum approved by the Division of Health Care Policy).
